    Economic potential of forest resources of Nepal

    Nepal's forest resources underpin the livelihoods of rural people in important ways. During the country's 'planned development' over the past 50 years, the government, donors and policy makers have viewed these resources as a key vehicle for ushering in economic growth and for meeting basic needs. They underscore the potential value of forest resources for achieving conservation and socio-economic objectives. To what extent have economic incentives been generated to effectively harness these resources to meet the said objectives is an open question. To address this question, this paper reviews briefly and broadly the economic potential of the country's forest resources in terms of forest goods and services. Estimates of economic potential of timber and non-timber forest products and environmental services have been assessed. A number of recommendations for realizing the potential for achieving development and poverty reduction objectives is provided

    Parafascicular Thalamic and Orbitofrontal Cortical Inputs to Striatum Represent States for Goal-Directed Action Selection

    Several lines of evidence accrued over the last 5-10 years have converged to suggest that the parafascicular nucleus of the thalamus and the lateral orbitofrontal cortex each represent or contribute to internal state/context representations that guide action selection in partially observable task situations. In rodents, inactivations of each structure have been found to selectively impair performance in paradigms testing goal-directed action selection, but only when that action selection relies on state representations. Electrophysiological evidence has suggested that each structure achieves this function via inputs onto cholinergic interneurons (CINs) in the dorsomedial striatum. Here, we briefly review these studies, then point to anatomical evidence regarding the afferents of each structure and what they suggest about the specific features that each contribute to internal state representations. Finally, we speculate as to whether this role might be achieved interdependently through direct PF→OFC projections, or through the convergence of independent direct orbitofrontal cortex (OFC) and parafascicular nucleus of the thalamus (PF) inputs onto striatal targets

    SIMULATION AND RESPONSE SURFACE METHODOLOGY TO OPTIMIZE WINTER WHEAT RESPONSE TO GLOBAL CLIMATE CHANGE

    Future climate changes can have a major impact on crop production. But, whatever the climatic changes, crop production can be adapted to climate change by implementing alternative management practices and developing new genotypes that will take full advantage of the future climatic conditions. Since the classical agronomic research approach is not possible in identifying these new agronomic technologies for the future climatic conditions, we used response surface methodology (RSM) in connection with the CERES-Wheat crop model and the HADCM2 climate simulation model to identify optimal configuration of plant traits and management practices that maximize yield of winter wheat under high CO2 environments. The simulations were conducted for three Nebraska locations (Havelock, Dickens and Alliance), which were considered representative of winter wheat growing areas in the central Great Plains. At all locations, the identified optimal winter wheat cultivar under high CO2 conditions had a larger number of tillers, larger kernel size, shorter days to flower, grew faster and had more kernels per square meter than the check variety under normal CO2 conditions, while the optimal planting dates were later and planting densities were lower than under normal conditions. We concluded that RSM used in conjunction with crop and climate simulation models was a useful approach to understanding the complex relationship between wheat genotypes, climate and management practices

    Pheochromocytoma: A Rare Cause of Secondary Hypertension

      Introduction: Pheochromocytoma is a rare tumor, originating from the chromaffin tissue. Its frequency is approximately 1/100,000. The clinical manifestation is due to catecholamine excess, which includes high blood pressure, palpitation, headache, sweating, nausea, vomiting, trembling, weakness, irritation, abdominal and chest pain, dyspnea, red warm face, constipation, polyuria, and polydipsia.   Case Report: We present a case of 53 years old male, hospitalized for hypertensive crisis following the manipulation of left sided frozen shoulder. He had labile blood pressure ranging from 220/120 systolic- 90/60 diastolic, profuse sweating and tachycardia. Findings of Contrast enhanced CT of abdomen was consistent with right adrenal pheochromocytoma and 24 hours urinary VMA was 17 mg /24 hr (Normal <13.6 mg /24hr). After the clinical, paraclinical investigations and radiological tests, it was proved to be a pheochromocytoma. The surgical intervention was planned. But due to unavailability of required antihypertensive drugs in Nepal (alpha-blockers like phentolamine and phenoxybenzamine), surgeons were reluctant to operate, although blood pressure was well controlled with use of sodium nitroprusside during hypertensive crisis and prazosin, a selective alpha blocker as maintainance therapy. The use of prazosin to control hypertension secondary to pheochromocytoma is limited to case report and case series.   Conclusion: Although rare, pheochromocytoma is a treatable surgical cause of secondary hypertension

    Physicochemical, antioxidant, and sensorial properties of peach snacks prepared from different cultivars

    High potentiality of medicinal benefits of peach have increased its demand, however, supply of fresh fruit to meet the demand is challenging as it grows in specific climatic regions and in particular season. Preparation of varieties of processed peach products could be a good option to supply in lean season. Objective of the study was to assess the quality characteristics of peach snacks prepared from 11 different cultivars. We investigated the physicochemical (soluble solid, titratable acidity, hardness and dry yield), antioxidant (DPPH radical scavenging capacity and total phenolics content) and sensorial properties (color, flavor, texture, sweetness and overall acceptance) of peach snacks prepared from 11 peach cultivars. Peach snacks of different cultivars evaluated in this study showed substantial variations in antioxidant capacity,  physicochemical and sensorial properties. Some of the samples showed higher physicochemical properties while the others contained better antioxidant capacity or sensory properties. Results of this study reveal that quality peach snacks having different properties could be prepared by drying thin slices of fresh fruits of different cultivars. However, supplementary studies on cost effective techniques of peach snacks preparation and variation in nutritive and medicinal properties of processed products could increase the application of the findings of this experiment.Key words: Antioxidant, peach snack, physicochemical, sensory property

    Effects of Resonant Cavity on Macroscopic Quantum Tunneling of Fluxon in Long Josephson Junctions

    We investigate the effects of high-Q_c resonant cavity on macroscopic quantum tunneling (MQT) of fluxon both from a metastable state to continuum and from one degenerate ground-state of a double-well potential to the other. By using a set of two coupled perturbed sine-Gordon equations, we describe the tunneling processes in linear long Josephson junctions (LJJs) and find that MQT in the resonant cavity increases due to potential renomalization, induced by the interaction between the fluxon and cavity.Enhancement of the MQT rate in the weak-coupling regime is estimated by using the experimantally accessible range of the model parameters. The tunneling rate from the metastable state is found to increase weakly with increasing junction-cavity interaction strength. However, the energy splitting between the two degenerate ground-states of the double-well potential increases significantly with increasing both the interaction strength and frequency of the resonant cavity mode. Finally, we discuss how the resonant cavity may be used to tune the property of Josephson vortex quantum bits.Comment: Accepted for publication in Phy. Rev.

    Upper Gastrointestinal Endoscopy in Lumbini Medical College and Teaching Hospital

      Introduction: Upper gastrointestinal (UGI) endoscopy includes visualization of the oropharynx, esophagus, stomach, and proximal duodenum, with real time assessment and interpretation of the findings encountered. An upper endoscopy is indicated in the diagnostic evaluation of signs and symptoms of a wide variety of gastrointestinal disorders. Besides there are some therapeutic implication of the endoscopy. This study was conducted to study the spectrum of diseases found during the upper gastrointestinal endoscopy in patient presenting in Lumbini Medical College and Teaching Hospital (LMCTH).   Methods: This was a retrospective observational study carried out in LMCTH. The endoscopic record book of the patients who underwent UGI endoscopy for various reasons from February 2011 to 2013 was analysed. The risk factor of smoking and alcohol was also included and analysed in the study.   Results: All together 550 upper GI endoscopy was performed in the two years. There were 290 males (52.72%) and females were 260 (47.38%). The mean age was 45.7 years (SD=17.9). Most of the patient belonged to the age group 41 to 80 years (71%). Among total patients, 209 (38%) of them were found to be macroscopically normal. Of those who had positive endoscopic findings; 165 (48.4%) had gastritis and 36 (10.6%) had duodenal ulcer, esophageal varices was in 30 (8.8%), gastric carcinoma in nine (2.6%) of cases. Cigarette smoking was significantly associated with the presence of peptic ulcer disease (p=0.01) and malignancy of gastrointestinal tract (p=0.03). Alcohol intake was non-significantly related to peptic ulceration (p=0.07) and malignancy of gastrointestinal tract (p=0.09).   Conclusion: Upper gastrointestinal endoscopy is a safe and useful procedure for investigating patients with gastrointestinal complains. Gastritis was the most common finding among the patients who had abnormal endoscopy followed by duodenal ulcer and esophageal varices

    Characteristics of Patients with Tuberculous Pleural Effusion in Rural Nepal

      Introduction: Tuberculosis (TB) is a major cause of mortality and morbidity in developing countries. TubercularPleural effusion is the second most common form of extra pulmonary tuberculosis (EPTB), superseded in Prevalence only by lymph node tuberculosis. Pleural effusion occurs in approximately 5% of patients with TB. The purpose of this study was to assess the demographic characteristics of patients presenting with pleural effusion in rural Nepal.   Methods: A retrospective study was conducted with all the cases diagnosed and admitted with pleural effusion at Lumbini Medical College And Teaching Hospital from April 2011 to March 2013 of all the cases diagnosed andadmitted with pleural effusion were included in the study. Hundred cases diagnosed with pleural effusion by clinical Examination or chest X-ray or ultrasonography’s (USG) of the chest were included in the studied. The following parameters patients demographic profile, causes of pleural effusion, location (unilateral/bilateral), hemoglobin and complete blood count, sputum stain and culture sensitivity, Monteux test, chest X-ray and USG findings and Pleural fluid analysis (biochemical, hematological, microbiological and cytological) were analyzed by using SPSS 21.   Results: Out of 100 cases, the cause of pleural effusion in 59 patients was tuberculosis, 14 by malignancy, next 14 by Para pneumonic Effusion, 12 by congestive cardiac failure and three cases by alcoholic liver disease. Patients with tuberculous pleural effusion were younger, predominantly males, had unilateral effusion, lower blood hemoglobin, lower Pleural fluid neutrophils, higher pleural fluid Adenosine Deaminase (ADA) levels and higher level of pleural fluid to serum protein ratio as compared to the patients with non-tuberculous effusion.   Conclusion: Tuberculosis is the most common cause of pleural effusion in patients of rural Nepal

    Illness Perception and Depressive Symptoms among Persons with Type 2 Diabetes Mellitus: An Analytical Cross-Sectional Study in Clinical Settings in Nepal

    Background. This study aimed to assess the relationship between illness perception and depressive symptoms among persons with diabetes. Method. This was an analytical cross-sectional study conducted among 379 type 2 diabetic patients from three major clinical settings of Kathmandu, Nepal. Results. The prevalence of depressive symptoms was 44.1% (95% CI: 39.1, 49.1). Females (p<0.01), homemakers (p<0.01), 61–70 age group (p=0.01), those without formal education (p<0.01), and people with lower social status (p<0.01) had significantly higher proportion of depressive symptoms than the others. Multivariable analysis identified age (β=0.036, p=0.016), mode of treatment (β=0.9, p=0.047), no formal educational level (β=1.959, p=0.01), emotional representation (β=0.214, p<0.001), identity (β=0.196, p<0.001), illness coherence (β=-0.109, p=0.007), and consequences (β=0.093, p=0.049) as significant predictors of depressive symptoms. Conclusion. Our study demonstrated a strong relationship between illness perception and depressive symptoms among diabetic patients. Study finding indicated that persons living with diabetes in Nepal need comprehensive diabetes education program for changing poor illness perception, which ultimately helps to prevent development of depressive symptoms
